Services Québec and the Directeur de l’état civil have entered into agreements with Service Canada (under the responsibility of Human Resources and Skills Development Canada), the Canada Revenue Agency and Aboriginal Afffairs and Northern Development Canada.
Under these agreements, the Directeur de l'état civil is authorized to forward these organizations birth-related information on behalf of parents who complete the forms Declaration of Birth and Application for Simplified Access to Birth-related Government Programs and Services so that they can apply for:
The Application for Simplified Access to Birth-related Government Programs and Services is accessible through the online electronic declaration of birth service and appended to the paper Declaration of Birth provided to parents by the hospital or birthing house when the child is born.
If parents do not complete the sections in the form concerning the Canada Revenue Agency, Service Canada and Aboriginal Affairs and Northern Development Canada, they must send applications to these organizations themselves and provide the required documents.
