Clientele Any person insured by the Québec health insurance plan which is in one of the following situations:
Reimbursement granted A person eligible for the program is entitled to the lump-sum amount of $200 for each breast to cover all or part of the costs related to the purchase of an external breastform.
Every 2 years thereafter, an eligible person is entitled to a lump-sum amount of $200 for each breast to cover the cost of replacing the breastform.
Supplement granted to recipients of last-resort financial assistance A person receiving last-resort financial assistance is entitled, upon presentation of proof of purchase, to a supplement (to a maximum of $100, if the cost of the breastform exceeds $200) to cover all or part of the actual purchase or replacement cost of a breastform.
To be entitled to this supplement, a person must be a recipient of last-resort financial assistance at the time of the mastectomy or the diagnosis of aplasia.
When replacing a breastform (mimimum of 2 years between each replacement), a woman receiving last-resort financial assistance is entitled to a supplement of $100 for each breast, provided she is still eligible for income support and she sends the original invoice to the Régie.
Note The Régie will send a cheque every 2 years to the address indicated in its files. It is not necessary to make a new application every 2 years, but the Régie must be notified of any change of address.
The External Breastforms Program started in January, 1978.
