Registering to the Québec Health Booklet

To sign up for the Québec Health Booklet service, a person must:
  1. Request a 4-digit security code from the Régie de l’assurance maladie du Québec (RAMQ) by using the service Access Your Health Info Online and providing his or her health insurance card number (12 characters) and postal code;
  2. Complete the signup process by using the Québec Health Booklet service’s page Complete Your Registration with clicSÉCUR--Individuals (the Québec government’s authentication service) to provide the security code received from the RAMQ.

If your RAMQ file is not up-to-date or if you are between the ages of 14 and 17, you must call the Régie to request your security code, which will be provided by email or by regular mail, as you prefer.

If you do not have a clicSÉCUR—Individuals account, you must sign up for one. You will be able to choose either one of the following options for access to:
  • The Québec Health Booklet only, by having your identify verified by the RAMQ (using your health insurance card number (12 characters); or 
  • All the online services offered by government departments and agencies who use clicSÉCUR, by having your identity verified by Revenu Québec (using your social insurance number, date of birth and a recent Notice of Assessment number received from Revenu Québec).
If you already have a regular clicSÉCUR—Individuals account, you can use it to access the Québec Health Booklet service.
 
Your security code is required only to register for the Québec Health Booklet service. It will not be required for subsequent visits.
 
Once you are registered, you can consult your personal health information by going to the Québec Health Booklet website.
