Claim Slip

Applying for a Claim Slip as a Non-Beneficiary of the Social Assistance Program, Social Solidarity Program or Aim for Employment Program

Follow the steps below to apply for a claim slip as a non-beneficiary of the Social Assistance Program, Social Solidarity Program or Aim for Employment Program.

  1. Complete the following forms:
    1. Application for Service - General Information
    2. Appendix 1 - Information about Education and Employment 
    3. Appendix 2 - Application for Last-Resort Financial Assistance 
    4. Estimation du coût des médicaments (in French only), if the individual does not have sufficient income to cover his or her medication needs 
  2. Enclose the required documents with the forms
  3. File the forms and documents in person at a local employment centre (CLE) or a Services Québec office that offers social solidarity services. You will be asked to provide 2 pieces of identification, including a photo ID. Note that you may also mail the forms and documents to a CLE or Services Québec office, but you will still be required to go there in person to provide the necessary identification documents. To find a CLE or Services Québec office, use the Local Employment Centre Locator, which is available online.

    Once the application has been assessed, you will be asked to provide certain documents pertaining to yourself, your spouse and your dependent children (see Definitions), if any.
