Notifying the Commission des normes, de l'équité, de la santé et de la sécurité du travail of the Death of a Person Who Has Applied for Reimbursement of Expenses Incurred as a Result of an Employment Injury

To notify the Commission des normes, de l'équité, de la santé et de la sécurité du travail of the death of a person who had applied for reimbursement of expenses incurred as a result of an employment injury, contact the Commission.
 
However, if the declarant of the death filled out the Application for the Simplified Forwarding of Information relative to the Death included in the declaration of death, the Directeur de l’état civil will inform the Ministère. In this event, you do not have to provide any proof of death to the Commission. You must, however, contact the Commission to find out what other procedures must be followed in regard to the death.
