The Québec Health Record: Refusal to Participate or Modification of the Refusal

A person can exercise his or her right to refuse to participate in the Québec Health Record in one of the following ways:
Once the refusal has been registered, a confirmation number is given to the person. If the refusal was made by mail, a written confirmation is sent to the address corresponding to the health insurance card number at the RAMQ in the days following the processing of the request.
Note If the person is incapacitated, the request must be made by his or her legal representative.
If the person is a child aged under 14, the request must be made by one of his or her parents.
A person can exercise his or her right to refuse to participate in the Québec Health Record at any time. Anyone who has refused to participate in the Québec Health Record may change his or her mind at any time and withdraw his or her refusal or change the end date of the refusal.
