Applying for the Special Benefit for the Purchase of Infant Formula

To apply for the special benefit for the purchase of concentrated liquid infant formula, the recipient of social assistance benefits or social solidarity benefits must proceed in one of the following ways, depending on the situation.

  • If the child is already included in the recipient’s file (proof of birth already provided), she must contact the Centre de communication avec la clientèle at the ministère du Travail, de l'Emploi et de la Solidarité sociale.
  • If the child is not already included in the recipient’s file, she must go to her local employment centre (CLE) or Services Québec office that provides social solidarity services with a copy of the Constat de naissance (attestation of birth). This form is completed by the doctor or midwife and a copy is given to the parents when a child is born.

Child of 9 months or older
If the child is aged 9 months or over, but less than 1 year old, the recipient must justify the need for infant formula by providing the Certificat médical (in French only) form completed by the doctor or midwife or by providing a written attestation from the doctor or midwife.
