Civil Status Documents Related to Marriage or Civil Union: Certificate, Copy of an Act or Attestation

Applying for a Certificate or a Copy of an Act of Marriage or Civil Union

To request a certificate or a copy of an act of marriage or civil union, use one of the following:

  • the online service DEClic! or the online service DEClic! Express
  • the form Application for a Certificate or Copy of an Act – Marriage or Civil Union
  • the counter service DEClic! Comptoir

Applying Through One of the Online Services
Use either DEClic! (clicSÉQUR - Citoyens authentication required) or DEClic! Express.

To be able to use DEClic! Express, you must have a rapid, secure access number (RSAN, or NARS in French). This number appears in the letter of confirmation mailed to spouses whose marriage or civil union has been recorded in the Québec register of civil status. It is valid for 90 days, or until the date indicated in the confirmation letter.

Applicants who use either DEClic! or DEClic! Express can then track the progress of their application using the online service Verification of the processing status of an online application.

Applying by Mail Using the Form
Fill out the form Application for a Certificate or Copy of an Act – Marriage or Civil Union and do one of the following:

  • Together with the documents and payment required, mail the form to the Directeur de l’état civil, in Québec.
  • Go to a service counter of the Directeur de l’état civil or one of the offices of Services Québec that offers services of the Directeur, present the form to the clerk along with the required documents, and make the required payment.

Applying through DEClic! Comptoir
To use DEClic! Comptoir:

  1. Go to a service counter of the Directeur de l’état civil, to a office of Services Québec that offers the service DEClic! Comptoir, or to the local employment centre for Baie-James.
  2. Present the required documents to the clerk.
  3. Make your payment.

The clerk will give you a transaction receipt with the file number for your application.

At the same location, you can then use a self-service Internet workstation to access DEClic! Comptoir and conclude your application, using the file number on the transaction receipt along with your postal code (in Canada or the United States).

If you prefer, you can conclude your application at any other location with Internet access. Note that you will have 3 days in which to do so, with a maximum of 3 attempts. After 3 days or 3 attempts, if you have still not completed the process you will need to reactivate your application. To do so, call the telephone number shown on the document you received with the transaction receipt.
