Applying for a Disabled Parking Permit

To apply for a disabled parking permit, proceed as follows: 

  1. Complete the Disabled Parking Permit Application for your type of vehicle (some sections may have to be filled out by a healthcare professional).
  2. Attach any required documents, together with the applicable payment.
  3. Mail to the Société de l’assurance automobile du Québec (SAAQ).

If  you always and permanently use a wheelchair to get around, attach proof that the purchase of the wheelchair was subsidized by the Régie de l’assurance maladie du Québec.

Note
At any time, the SAAQ may ask the holder of a disabled parking permit to complete a new assessment form, or to have additional assessments done, to ensure that he or she still meets the eligibility requirements.
