Correcting Information on a Health Insurance Card

To have the information on your health insurance card corrected, proceed as follows:

  1. Call the Régie de l’assurance maladie du Québec to request the Identity Correction form or obtain one at a Régie service point
  2. Complete the form and attach the Notice of Renewal form sent by the Régie, as well as any other required documents, if applicable 
  3. Submit the form and all accompanying documents in person at a Régie service point
    (in particular, a CLSC or a Régie office) or send by mail if you are exempt from providing a photo or signature
To find a Régie service point, consult the Service points section of their website.
A correction to a last name, first name, date of birth or sex designation requires that a new card be issued. If you plan to consult a health professional and have not yet received your new health insurance card, contact the Régie beforehand.
