Replacing a Lost, Damaged or Stolen Health Insurance Card

Follow one of the procedures below to replace a lost, damaged or stolen health insurance card.

To Replace Your Health Insurance Card Only
  • Use the online service Replacement of a lost, stolen or damaged health insurance card (ClicSÉQUR – Citoyens authentication required) and pay the fee (if applicable), or
  • Complete the Remplacement de carte form (to obtain the form, see below), attach the fee (if applicable) and either:
      • mail the form and payment to the Régie de l’assurance maladie du Québec, or 
      • submit the form at a Régie service point (in particular, a CLSC or a Régie office)

To obtain the Remplacement de carte form, either:

  • use the Régie’s online service, Order a form to renew my Health Insurance Card or to replace my lost, damaged or stolen card, or
  • call the Régie

The photo and signature from your original card are generally reused.

To Replace Your Health Insurance Card and Your Driver’s Licence
If you wish to replace both your health insurance card and driver’s licence, you can either:

  • use the online service Replacement of a lost, stolen or damaged health insurance card (ClicSÉQUR – Citoyens authentication required) and pay the fee (the Régie will send you the card automatically), and go to a Société de l’assurance automobile du Québec service outlet to have your driver’s licence replaced (other fees will be payable to the Société), or 
  • go to a Société service outlet to replace both your health insurance card and driver’s licence at the same time (you will be provided with the card replacement form onsite) and: 
      • provide 2 pieces of ID (your photo will be taken onsite and used for both your card and your licence) 
      • pay the required fee

To obtain the Remplacement de carte form, you can either:

  • use the online service Order a form to renew my Health Insurance Card or to replace my lost, damaged or stolen card, or
  • call the Régie

Note
If you plan to consult a health professional and have not yet received your new health insurance card, contact the Régie beforehand.
