Replacing a Lost, Stolen or Deteriorated Health Insurance Card

To replace a lost, stolen or deteriorated health insurance card, you can either:
  • use the Replacement of a Lost, Stolen or Deteriorated Health Insurance Card online service (clicSEQUR identification required), or
  • telephone the Régie de l’assurance maladie du Québec to obtain a copy of the card replacement form and send the completed form to the Régie
The photo and signature from your original card are generally reused.
 
Replacing Your Health Insurance Card and Driver’s Licence at the Same Time
To replace your health insurance card and driver’s licence at the same time, you can either:
  • use the Replacement of a Lost, Stolen or Deteriorated Health Insurance Card online service (clicSÉQUR authentication required) to replace your health insurance card, and then go to a Société de l’assurance automobile du Québec service outlet to have your driver’s licence replaced, or
  • go to a Société service outlet to replace your card and licence at the same time and provide 2 pieces of ID (your photo will be taken on-site and used for both your card and your licence)
