Applying for a Death Certificate or a Copy of an Act of Death

You can apply for a death certificate or a copy of an act of death in the following ways:

  • Online using the DEClic! service
  • By completing the form entitled Application for a Certificate or Copy of an Act – Death 
  • In person using the DEClic! Comptoir service
Online Application using the DEClic! Service
To apply online, you must use the DEClic! service (clicSÉQUR - Citoyens authentication required).
 
You can then track the progress of your application using the online service Verification of the Processing Status of an Online Application.
 
Application using the Form
After completing the form entitled Application for a Certificate or Copy of an Act – Death, either:
  • mail the form, payment and required documents to the Directeur de l'état civil office in Québec, or
  • go to a Directeur de l'état civil service counter or a Services Québec office that offers Directeur de l'état civil services, present the form to the clerk with the specified documents and make the required payment
Application in Person using the DEClic! Comptoir Service
To apply in person:
  1. Depending on where you live, you must go to either:
    1. a Directeur de l'état civil service counter, or 
    2. a Services Québec office that offers DEClic! Comptoir services, or 
    3. the local employment centre in Baie-James
  2. Present the specified documents to the clerk
  3. Make the required payment
The clerk will give you a transaction receipt indicating a file number.
 
At the same location, you can then use a self-service Internet workstation to access the DEClic! Comptoir service and finalize your application using the file number on the transaction receipt and your postal code (in Canada or the United States).
 
If you prefer, you can finalize your application at any other location with Internet access. Note that you will have 3 days in which to do so and a maximum of 3 attempts. If you do not finalize your application within 3 days or 3 attempts, you must reactivate your application by calling the telephone number shown on the document you received with the transaction receipt.
