There are 2 ways to apply for benefits:
Applying for benefits onlineThen, he or she must complete an application for benefits according to his or her personal situation and the type of plan selected.
If the parent does not complete the application for benefits all at once, he or she does not have to start the process over from the beginning. The information already provided can be saved for a maximum of 30 days.
Once the application submitted, the parent will automatically receive a number confirming that the application has been registered with the QPIP.
The required documents (originals only) must be mailed to the QPIP. They will be returned to the parent after. The parent can file an application for benefits even if he or she does not have his or her record of employment.

Applying for benefits by phone It is possible to file an application for benefits over the phone by calling the QPIP’s Centre de service à la clientèle.
An officer will complete the application for benefits based on the information provided by the parent and then mail him or her the completed form to sign.
The parent can file an application for benefits even if he or she does not have his or her record of employment.
Even if the parent applied for benefits by phone with the help of an officer from the QPIP's Centre de service à la clientèle, he or she can, at any time, register for the QPIP's online services to consult and manage his or her file online.
The earliest a parent can apply for benefits is the calendar week (from Sunday to Saturday) during which he or she wishes to start receiving benefits.
For example, if the parent stopped working on Friday and wishes to start receiving benefits the following week, he or she will have to wait until Sunday to file the application. If the parent files his or her application on the Friday (the last day of work), payment of benefits will begin the week beginning on the previous Sunday.
The date the application is sent is considered the date on which the application was filed.
Wage earner If the parent is a wage earner, he or she can apply for benefits as soon as he or she stops working. The parent must submit your the application on the day he or she stops working or as of the first day his or her regular weekly earnings are reduced by at least 40%.
Self-employed worker If the parent is a self-employed worker, he or she can apply for benefits on the day he or she ceases business activities or as of the day he or she reduces the time spent on business activities by at least 40%.
Worker receiving indemnities further to precautionary cessation of work If the parent is pregnant and receiving indemnities from the Commission des normes, de l'équité, de la santé et de la sécurité du travail further to precautionary cessation of work, she may apply for benefits under the QPIP up to four weeks prior to the expected week of birth. If she is eligible for QPIP benefits, the Commission will cease paying her indemnities as of the date of application for QPIP benefits.

Decisions are usually rendered within 14 days following the date the QPIP’s Centre de service à la clientèle received all documents needed to study an applicant’s file.
Only original documents are accepted. They will be returned to the applicant.
