For a child to be eligible for the public health insurance plan, the parent with whom he or she permanently lives must be recognized as a:
Parents who have completed the forms Declaration of Birth and Application for Simplified Access to Birth-related Government Programs and Services do not need to take any steps to obtain their child’s health insurance card. After receiving the forms, the Directeur de l'état civil forwards birth-related information to the Régie de l'assurance maladie. If the child is eligible, the Régie registers the child for the health insurance plan and issues a health insurance card.
The undertakes to send the health insurance card within 10 working days of receiving the information forwarded by the Directeur de l’état civil.
