Filing a Monthly Statement for the Social Assistance Program

To file your monthly statement for the Social Assistance Program, fill out the Monthly Statement form that you received with your cheque or deposit notice and send it to your local employment centre (CLE) or Services Québec office that provides social security services by fax or mail. You can also remit it in person.
 
However, if you are an adult without a spouse or are the head of a single-parent family and have no change to declare, you can file your statement by telephone using the monthly statement telephone system. Be sure to have your Monthly Statement form, and provide the required information.
 
You can find the access code needed to file your monthly statement by telephone on the form. If you lose the form, you will have to fill out a paper form and remit it to your local employment centre (CLE) or Services Québec office providing social security services.
