To notify Revenu Québec of a change of address in connection with the work premium, proceed in one of the following ways:
- Use the Service québécois de changement d’adresse, either online or with the help of a Services Québec information officer (by telephone or at a service counter).
- Use the online service Change Address on Revenu Québec’s website.
- Notify Revenu Québec of your new address by telephone or postal mail.
However, if you receive social assistance payments and applied for advance payments of the work premium through the Ministère du Travail, de l’Emploi et de la Solidarité sociale, you must notify the Ministère of your change of address. To do so, proceed in one of the following ways:
- Use the Service québécois de changement d’adresse, either online or with the help of a Services Québec information officer (by telephone or at a service counter), and provide the information required.
- Fill out the detachable flap on the return envelope you received with your Monthly Statement form (included with your cheque or deposit notice), and either mail it to the Ministère or present it at the local employment centre (CLE) for your area.
- Call the Centre de communication avec la clientèle at the Ministère.

The processing time varies depending on the method you used to indicate your change of address.
Information Required
If you use the Service québécois de changement d'adresse or Revenu Québec’s Change Address online service, without going through My Account, you must provide the following information:
- the amount on line 199 of your income tax return for the last year for which you received a notice of assessment
- your former postal code
- your former and new addresses
