Any worker, or person considered as such by the Commission des normes, de l'équité, de la santé et de la sécurité du travail, having sustained an employment injury because of which expenses were incurred.
Various terms and conditions apply when the employment injury occurs in a border region or outside Québec.
The authorization of the Commission is required in a number of situations, particularly for reimbursement for travel and living expenses.
To apply for the reimbursement of expenses incurred because of an employment injury, the worker must first, within six months following the onset of the injury:
If the application concerns drugs, travel, meals or living expenses, or expenses for the cleaning, repair or replacement of clothing, the worker must next:
- complete the Worker’s Claim form, enclose the required documents and mail the form and documents to the office of the Commission des normes, de l'équité, de la santé et de la sécurité du travail in the worker’s region, after being sure to photocopy the form; or
- use the Réclamation du travailleur online service and submit the form, after being sure to print it (or call the Commission to obtain a copy by mail), then mail in the documents required.
- complete the Application for Reimbursement of Expenses form, enclose the required documents, then mail the form and documents to the regional office after being sure to photocopy the form; or
- use the Demande de remboursement de frais online service for reimbursement of expenses service and submit the form after being sure to print it (or call the Commission to obtain a copy by mail), then mail in the documents required before submitting the form.
It is important that the worker keep a copy for his or her records of all the documents sent and another copy to be submitted to the worker’s employer.
It is preferable to contact the Commission before travelling more than 100 kilometres, in order to find out whether it will authorize reimbursement of the cost.
