Applying for Social Solidarity Benefits

To apply for social solidarity benefits, a person must 
  • complete the following forms:
    • Application for Service - General Information,
    • Appendix 1 - Information about Education and Employment Services,
    • Appendix 2 - Application for Last-Resort Financial Assistance;
  • have a physician complete the Rapport médical (Medical Report) form;
  • bring the forms and 2 pieces of identification (one with a photo) to a local employment centre (CLE) or a Services Québec office that provides social solidarity services.

After your application is assessed, you will have to provide certain documents about yourself, and about your spouse and dependent children, if any.

Note
You can also mail the forms to your local employment centre (CLE) or Services Québec office. However, once they are received you will have to go in person to the CLE or Services Québec office so that your identity can be verified.
