Applying for Social Solidarity Benefits

To apply for social solidarity benefits, a person must 
  • complete the following forms:
    • Application for Service - General Information,
    • Appendix 1 - Information about Education and Employment Services,
    • Appendix 2 - Application for Last-Resort Financial Assistance;
  • have a physician complete the Rapport médical (Medical Report) form;
  • bring the forms and 2 pieces of identification (one with a photo) to a local employment centre (CLE) or a Services Québec office that provides social solidarity services.

The person can also mail the forms to the local employment centre (CLE) or Services Québec office. However, once they are received he or she will have to go to the CLE or Services Québec office in person so that his or her identity can be verified.

After the application is assessed, the person will have to provide certain documents concerning him or herself and his or her spouse and dependent children, if any.
