Applying for Social Solidarity Benefits

To apply for social solidarity benefits, proceed as follows:

  • Complete the following forms:
      • Application for Service – General Information
      • Appendix 1 – Information about Education and Employment Services
      • Appendix 2 – Application for Last-Resort Financial Assistance
  • Have a physician complete the Rapport médical (medical report) form.
  • Bring the forms and the required pieces of identification to a local employment centre (CLE) or a Services Québec office that provides social solidarity services. Your spouse and dependent children, if any, will also have to present the required identification.
 
In addition, your spouse must fill out the Application for Service - General Information and Appendix 1 - Information about Education and Employment Services forms, as well as the section of Appendix 2 - Application for Last-Resort Financial Assistance form that is reserved for spouses.
 
After your application is reviewed, you will have to provide certain documents for yourself and for your spouse and dependent children, if any.
 
Note
You can also mail the forms to your local employment centre (CLE) or a Services Québec office. However, once they are received you will have to go to the CLE or Services Québec office in person to submit the required pieces of identification.
