Applying for Financial Assistance under the Road Vehicle Adaptation Program for People with Disabilities

To apply for financial assistance under the road vehicle adaptation program for people with disabilities:

  1. Telephone the Service de la gestion des fournisseurs at the Société de l'assurance automobile du Qubec to request a copy of the Demande d'aide financière pour l'adaptation d'un véhicule form and find out how to have an occupational therapist assess your condition.
  2. Complete the form, attach a recent medical certificate attesting to your impairment, and send both documents to the SAAQ in the return envelope provided.
  3. Consult an occupational therapist for an assessment of the physical disability that prevents you from driving or getting into or out of a vehicle and for a recommendation regarding the equipment you require.
  4. Mail the occupational therapist's functional assessment report to the Service de la gestion des fournisseurs at the Société de l'assurance automobile du Québec, or ask your occupational therapist to send it.

If you hold a driver's licence, you must also provide a copy of the occupational therapist's functional assessment to the Service de l'évaluation médicale et du suivi du comportement at the SAAQ, since the addition of equipment may affect the class or conditions or your driver's licence.

After reviewing your application and in the event of a favourable decision, the SAAQ will confirm in writing the maximum amount to which you are entitled and enclose a list of authorized adaptation equipment. Depending on the amount of work to be done , you will have to send one or two estimates from vehicle adaptation suppliers to the Service de la gestion des fournisseurs.
