Registering your Consent to Organ or Tissue Donation after Death

Anyone who has reached the age of majority.

Minors cannot be registered for organ and tissue donation in the Registre of the Chambre des notaires, even at the request of the parents or guardian. However, this does not prevent organ and tissue donation if the parents give consent when the situation arises.

Anyone who is at least 8 years of age.

To indicate your consent using the consent sticker included in the publication Sign for Life!, simply sign the sticker and apply it to the back of your health insurance card. However, the consent sticker of a donor who is under 14 must be signed by his or her parent or guardian.

To have your consent recorded in the Registre des consentements au don d’organes et de tissus of the Chambre des notaires, indicate your wishes to your notary when drawing up your will or protection mandate.

To register yourself or your child in the Registre des consentements au don d’organes et de tissus of the Régie de l’assurance maladie du Québec, fill out the Consent to Organ and Tissue Donation form that you received in the mail when your health insurance card was renewed. Other ways of obtaining the form:
  • telephone or go in person to a reception desk of the Régie
  • download a personalized form through the online service Consent to Organ and Tissue Donation (requires clicSÉQUR authentication)

The form must then be sent in by mail or presented at one of the following:

  • a reception desk of the Régie
  • a local community service centre (CLSC), when renewing your health insurance card or registering for the health insurance plan
  • a service outlet of the Société de l’assurance automobile du Québec, when renewing your health insurance card and driver’s licence

If the donor is under 14, the consent form must be signed by his or her parent or guardian.

You should notify your family that you want to donate your organs and tissues, to ensure that your wishes are respected in the event of your death.
