Notifying Consent for an Organ or Tissue Donation after Death

If a person wishes to donate organs or tissue after death, he or she may consent to do so in one of the following ways: 
  • sign the consent sticker and affix it to the back of the health insurance card; 
  • register the decision in the Registre des consentements au don d'organes et de tissus of the Chambre des notaires when drawing up a will or protection mandate; or
  • register in the Registre des consentements au don d'organes et de tissus of the Régie de l'assurance maladie du Québec and sending the Consent to organ and tissue donation form to the Régie;
This form is provided with the renewal notice of the health insurance card and can be obtained from the Régie de l'assurance maladie du Québec. It is also possible to download it by using the Consent to organ and tissue donation online service (mandatory authentication by clicSÉQUR).

The person must notify family members about his or her desire to donate organs or tissue so that this wish will be respected in the event of his or her death.
 
Upon the death of the person, his or her relatives will have to complete a medical/social questionnaire about the deceased.
