Applying for Reimbursement of Expenses Related to Death from an Employment Injury

Clientele
Any person who covered the deceased employee’s funeral expenses and the cost of transporting the body is entitled to a reimbursement.

Condition
The death of the employee must have been caused by an employment injury (see Definitions).

To apply for the reimbursement of expenses related to death from an employment injury, follow one of the following procedures:
  • Complete the Application for Reimbursement of Expenses form, and mail it along with the original invoices and receipts mail it to the regional office of the Commission des normes, de l'équité, de la santé et de la sécurité du travail, after making a photocopy of the form.
  • Use the Demande de remboursement de frais online service, and submit the form, after printing it out (or call the Commission to request a copy by mail), and mail the original invoices or receipts to the Commission.

It is recommended to keep a copy for your records.
