Reporting a Lost or Stolen Death Certificate or Copy of an Act of Death to the Directeur de l’état civil

Any person who wants to report a lost or stolen death certificate or copy of an act of death to the Directeur de l’état civil.

The loss or theft of a death certificate or a copy of an act of death must be reported to the Directeur de l'état civil before applying for a new one. The loss or theft will be registered in Québec register of civil status in order to invalidate the lost or stolen document and prevent identify theft.

Note
The documents issued by the Directeur de l’état civil should not be carried but kept in a safe place.
