To apply for the special benefit for the purchase of infant formula, the mother or the person responsible for the child must proceed in one of the following ways, depending on the situation.
If the child is already on file (proof of birth already provided), contact the Centre de communication avec la clientèle at the Ministère du Travail, de l'Emploi et de la Solidarité sociale.
If the child is not on file, go to your local employment centre (CLE) with your copy of the Constat de naissance (attestation of birth). This form is completed by the doctor or midwife and a copy is given to the parents upon the birth of the child.
You must inform your agent at the CLE of your choice to feed the child with infant formula in the form of liquid concentrate.
Child of 9 months or older
If the child is 9 months or older, but less than 1 year old, the mother or the person responsible for the child must provide a medical certificate from a doctor or midwife justifying the need for infant formula. You may use the Certificat médical form or provide a certificate written by the doctor or midwife.
