To notify Revenu Québec of the death of a recipient of the tax credit for home-support services for seniors, the liquidator of the succession (estate) must contact Revenu Québec and provide the death certificate and the document(s) proving the identity of the person designated as liquidator.
Revenu Québec must be notified of the death as soon as possible.
The required documents are
- the death certificate;
- the document(s) proving the identity of the person designated as liquidator of the succession, such as
- a certified copy of the notarial will, any related codicil and the will search certificates issued by the Chambre des notaires du Québec and the Barreau du Québec,
- a certified copy of the holograph will or the will made in the presence of witnesses, a certified copy of the judgment probating the will or the minutes of the probate, as well as the will search certificates issued by the Chambre des notaires du Québec and the Barreau du Québec,
- a letter signed by the heirs designating the liquidator,
- the original or a certified copy of the court judgment,
- the original or a certified copy of the marriage contract,
- the original or a certified copy of a declaration of heredity recorded by a notary.
