Change of mailing address
Workers can notify the Commission de la construction du Québec of a change in their mailing address, that is the address where they receive their mail, in one of the following ways:
- by sending a duly completed copy of the Formulaire de changement d'adresse (in French only) by mail or by fax to the customer service of the Commission office in their region;
- by calling the customer service of the Commission office in their region;
- by visiting the customer service of the Commission office in their region;
- by using the Commission's change of address online service (in French only).
Change of residential address
Holders of a competency certificate or of an exemption who wish to notify the Commission of a change in their residential address must complete the Formulaire de changement d'adresse (in French only). They must enclose with the form 3 documents proving that they reside at their new address and send all the documents to the customer service of the Commission office in their region either by mail or by fax, or bring them in person.
Workers who do not hold a competency certificate or an exemption can notify the Commission of a change in their residential address in one of the following ways:
- by sending a duly completed copy of the Formulaire de changement d'adresse (in French only) by mail or by fax to the customer service of the Commission office in their region;
- by calling the customer service of the Commission office in their region;
- by using the Commission's change of address online service (in French only).
Documents that can be provided to prove residence include the following:
- a bill from a service provider (such as electricity, cable, gas or telephone bills);
- a credit card statement;
- a municipal tax bill;
- a car insurance certificate;
- a home insurance certificate;
- a document issued by the Government of Canada (such as a notice of assessment issued by the Canada Revenue Agency);
- a document issued by the gouvernement du Québec (such as a notice of assessment issued by Revenu Québec or a driver's license issued by the Société de l'assurance automobile du Québec);
- an investment statement;
- a house purchase contract;
- a letter from a union association stating that the change of address has been made with the association.
The documents provided must bear the person's name and new address. Original documents, photocopies or printouts of electronic bills are accepted.
