Registering a Child Born in Québec for the Public Health Insurance Plan

Children born in Québec.

For a child to be eligible for the public health insurance plan, the parent with whom he or she permanently lives must be recognized as a: 

  • Person who has settled in Québec
  • Person temporarily staying in Québec.

For an enfant to be registered under the public insurance plan, the parents must complete one of the following:
  • The Application for Simplified Access to Birth-related Government Programs and Services forms, using the online Electronic Declaration of Birth; or
  • The corresponding paper declaration of birth, found in the document provided by the health institution or birthing centre.

fter receiving the forms, if consent is provided, the Registrar of Civil Status forwards birth-related information to the Régie de l'assurance maladie (RAMQ) for registration of the child under the public health insurance plan. If the child is eligible, the Régie registers the child for the health insurance plan and issues a health insurance card.
